State Network of Colorado Ambulatory Practices & Partners ~ An affiliation of Colorado practice-based research networks (PBRNs) sponsored by the University of Colorado Department of Family Medicine ~ SNOCAP is an umbrella organization of three Colorado primary care networks —High Plains Research Network (HPRN), Colorado Research Network (CaReNet), and Building InvestiGative Practices for Better Health Outcomes Network (BIGHORN)— created to facilitate resource-sharing and collaboration. A network of private practices associated with the Colorado Academy of Family Physicians created to enhance health care for the people in its communities. Director: Peter Smith, MD BIGHORN home   A statewide network of community health center clinics, residency training sites, university-affiliated private practices, and a patient advisory council with a particular focus on disadvantaged populations. Director: Bennett Parnes, MD CaReNet home   A rural network of physician offices, hospitals, nursing homes, hospice organizations, home health agencies, public health offices, and community members in eastern Colorado.
