Application Requirements
Applicant must have completed a Family Medicine, Internal Medicine, or an Emergency Medicine residency program prior to fellowship. Our fellowship participates with the National Resident Matching Program (NRMP) through American Medical Society for Sports Medicine (AMSSM). Please send the following items with your application:
- A completed AMSSM application form (can be accessed at
www.amssm.org)
- A current CV
- Three letters of reference
- A letter from your current residency program director indicating
your anticipated completion date and standing within the residency
- A copy of your medical school Dean’s letter/Program Evaluation

- A copy of your medical school transcript
- Copies of your USMLE scores
- A copy of your current medical license
- Residents in our program must be a U.S. citizen,
lawful permanent resident, refugee, asylee, or
possess the appropriate documentation to allow
Resident to legally train at the University of
Colorado Denver School of Medicine.
We participate in the National Fellow Match Program. For academic year 2011-2012. We will begin accepting applications September 1, 2010 and a complete application is due by September 24, 2010. Send all applications to:
