ASIPS issued a number of patient safety alerts when we recognized significant threats to patient safety that were reported to our system. You can view each alert below.

Alert 1-2002:

"What do you mean I'm not immune?" Adult Patients Receiving Pediatric Doses of Immunizations

ASIPS Alert 2-2002:

"High Glucose Level? I didn't notice it!" Some laboratories don't flag abnormal random glucoses on reports
